Housing-Related Forms

Forms Available on the Housing Portal
All 51³Ô¹ÏºÚÁÏ students who have applied to live in the residence halls can request to cancel their housing and dining contract by contacting the Office of Housing & Residential Life or finding the contract release form on their housing portal. The cancellation fees apply for all reasons except for non-enrollment. Any documentation related to your reasoning may increase the likelihood of an approved contract release.
New and returning residents must submit their contract release forms by the date listed on the Housing portal. After that date, a cancellation fee will apply until move-in day. After move-in, the contract can be terminated for a buyout fee as described in the contract.
If you graduated high school outside of Clark County, you may have a first-year on-campus housing requirement. In order to remove this, you must complete a request for exemption. When completing this form, please make sure to read the front page in its entirety.
In order to be exempt from living on-campus, you must fall under one of the reasons listed on the form. Please provide all necessary documentation listed on the first page. Without this, your request will be denied.
Any housing and associated dining plan charges can be disputed by completing a Billing Appeal Form.
Upon using your meal plan, if you find yourself wanting to change to another meal plan you will need to complete a form to make this change. These changes can be made during the first three weeks of classes only.
Winter stay forms are for students who will remain in the residence halls over winter break. Forms to request staying over break will typically be available starting in November and will close by January.
Summer housing applications typically start in April and will roll through the entire summer until the beginning of august. Refer to the for the most up-to-date information and to complete the Summer Housing Application. Please contact our office.
If you need to stay past the end of the semester, please fill out a Late Stay Request Form that can be found either in your resident housing portal or by visiting the Housing Front Desk.
FERPA
Do you want to know more about how your student is doing at 51³Ô¹ÏºÚÁÏ? Need information about his or her housing account balances? Housing & Residential Life encourages students to keep in touch with their families and friends while they experience living independently.
Due to the Family Education Rights to Privacy Act (FERPA) of 1974, 51³Ô¹ÏºÚÁÏ cannot disclose student records to anyone without the student's permission, including parents and legal guardians. The student must complete a FERPA release through their My51³Ô¹ÏºÚÁÏ account and/or their to give someone other than the student permission to discuss the student's account with Housing & Residential Life. The information provided on the Housing Portal is for Housing & Residential Life use only. We strongly recommend adding FERPA release information to your My51³Ô¹ÏºÚÁÏ account in addition to the Housing Portal.
This will allow a third party to obtain information only. All decisions about meal plans, contract changes, or other special requests must still be communicated directly by the student to Housing & Residential Life. All Housing & Residential Life account statements and other correspondence will be emailed to the student's RebelMail account.

Canceling Your Room or Housing Contract
When you sign up for housing at 51³Ô¹ÏºÚÁÏ, you’re agreeing to a year-long contract for both fall and spring semesters. If you sign after the academic year starts, the contract still lasts until the end of spring. Cancellation fees apply regardless of your reason, except if you’re not enrolling at 51³Ô¹ÏºÚÁÏ.
To cancel your housing and dining contract, you need to contact the Office of Housing & Residential Life or use the release form on your . We also encourage you to view our Important Dates page, as cancellation fees change after specific dates.
